for all health care organizations in the Institute of Medicine’s 2000 report, To Err is Human: Building a Safer Health System.
Medication reconciliation offers a super opportunity to improve safety in hospitals. This learning session will provide
valuable tools for directly enhancing patient safety in your facility.
